Wasatch Peak Family Practice Contributing Transcription Data to the CHIE!

UHIN is pleased to announce that Wasatch Peak Family Practice began sharing transcription information with the cHIE on November 30, 2016. Wasatch Peak is a four-physician clinic in Layton that offers general and preventive care for children and adults. They’ve been contributing laboratory and radiology data since July. With the addition of the transcription feed, Wasatch Peak is now also contributing valuable information such as office notes and history and physical reports. Thanks to Wasatch Peak Family Practice for their dedication to improving care coordination!

The CHIE Welcomes Beaver Valley Hospital and Milford Valley Memorial Hospital!

UHIN is pleased to announce the CHIE’s newest data sources, Beaver Valley Hospital and Milford Valley Memorial Hospital! Beaver Valley Hospital is a 49 swing-bed medical center in Beaver City, Utah, offering surgical, birthing, emergency and long-term care services. Beaver Valley Hospital, the nearby 25-bed Milford Valley Memorial Hospital, and three medical clinics form the Beaver Area Healthcare Network. In addition to providing acute care and family medicine, the healthcare network actively encourages disease prevention and healthy living in the community.

Beaver Valley Hospital and Milford Valley Memorial Hospital are contributing laboratory results and radiology reports to the CHIE’s Clinical Portal. The two hospitals are also contributing Admit, Discharge and Transfer (ADT) messages to the CHIE Alerts hospital encounter notification system.

CHIE participation by rural healthcare facilities helps ensure that patients throughout Utah receive the care they need, when they need it while traveling or seeking specialty care away from home. UHIN congratulates Beaver Valley Hospital and Milford Valley Memorial Hospital for their commitment to quality patient care.

The CHIE is continually growing, closing information gaps for patients in more and more areas. The Clinical Portal now includes data from 448 clinics and 45 hospitals in Utah and surrounding states. The CHIE Alerts system sends notifications from 24 Utah hospitals, as well as 41 hospitals in Colorado and Arizona through the Patient-Centered Data Home (PCDH) project. Tanner Clinic Becomes the CHIE’s Latest Data Source!

The CHIE is now receiving Continuity of Care Documents, CCDs, from Tanner Clinic! Tanner Clinic has locations in Kaysville, Layton, Roy, and Syracuse, and offers care in specialties including family medicine, pediatrics, OB/GYN, podiatry, sports medicine, orthopedics, endocrinology, urgent care and more. Tanner Clinic will automatically send CCDs, also called patient summaries, to the CHIE for all of their patients. Simply search for your patients in the Clinical Portal to find Tanner Clinic data in the Patient Summary view, whenever you need it. Additional details, such as the text of provider notes, may also be available under the “Documents” tab. To view these details, click on the CCD you’d like to view and choose the HTML option.

Uintah Basin Medical Center Joins the CHIE

UHIN is pleased to welcome Uintah Basin Medical Center to the CHIE! As the CHIE’s newest data source, the 49-bed hospital in Roosevelt, Utah is utilizing CHIE Alerts, to notify providers when their patients have encounters at Uintah Basin’s inpatient facility or emergency department. Additionally, as of August 29, 2016, Uintah Basin contributes general laboratory information to the Clinical Portal, and is actively working with UHIN to send radiology reports and transcription notes in the near future.

As the only hospital within 25 miles of Roosevelt, Uintah Basin plays a crucial role in coordinating care and enabling seamless hospital-to-home transitions for area residents. CHIE Alerts enable providers to follow up with their patients in a timely manner after discharge, reducing the risk of readmissions and complications. UHIN is working with Uintah Basin Medical Center and area clinics to make transitions of care smoother by connecting more clinics to receive CHIE Alerts and locate patient data in the Clinical Portal.

Current users of CHIE Alerts can receive Uintah Basin’s Alerts without any changes to your existing setup. In addition to Uintah Basin, CHIE Alerts are now available from 18 other hospitals throughout Utah, as well as 27 Colorado hospitals and 14 Arizona hospitals through the Patient Centered Data Home initiative, allowing information to follow the patient no matter where they are in the care continuum.

The CHIE Grows int Central Utah With Canyon View Medical Group!

The CHIE is on a roll this summer, with 55 new data sources coming aboard between June and August! The latest to join is Canyon View Medical Group, which is contributing Continuity of Care Documents, CCDs, to the CHIE Clinical Portal as of August 10, 2016. Canyon View Medical Group includes four family medicine clinics, two orthopedics clinics, two pediatrics clinics, and three women’s care clinics, all located throughout Utah Valley. Canyon View will automatically send CCDs, also called patient summaries, to the CHIE for all of their patients.

Simply search for your patients in the Clinical Portal to find Canyon View data in the Patient Summary view, whenever you need it. Additional details, such as the text of provider notes, may also be available under the “Documents” tab. To view these details, click on the CCD you’d like to view and choose the HTML option.

41 More Clinics Contributing Patient Summaries to the CHIE!

UHIN is excited to announce that Continuity of Care Documents, CCDs, from Physician Group of Utah (PGU), Rocky Mountain Women’s Health Center, and Heart and Lung Institute clinics are available in the CHIE Clinical Portal as of August 1, 2016! The 41 practices contributing CCDs offer care in Davis, Salt Lake and Tooele counties, in specialties including family medicine, cardiology, endocrinology, obstetrics and gynecology and orthopedics. They will automatically send CCDs, also called patient summaries, to the cHIE for all of their patients.

Simply search for your patients in the Clinical Portal to view PGU data in the Patient Summary view, where the data will remain available. Additional details, such as the text of provider notes, may also be available under the “Documents” tab. To view these details, click on the CCD you’d like to view and choose the HTML option.

CHIE Data Now Available From Cache Valley Hospital!

Cache Valley Hospital (formerly called Cache Valley Specialty Hospital) is once again contributing data to the CHIE as of March 14, 2016.

Cache Valley is a long-time CHIE supporter. The hospital initially began contributing data to the CHIE in 2013. Upon becoming part of HCA/MountainStar in 2015, they switched to HCA’s health information technology system. Cache Valley is now contributing laboratory, radiology and transcription reports, as well as sending CHIE Alert notifications when a patient has an encounter at their facility.

The 22-bed hospital in Logan offers a range of specialty treatments, acute care and rehabilitation services.

Cache Valley Hospital also served as the pilot provider when the CHIE was connected to send immunization data to the Utah Statewide Immunization Information System (USIIS) in November 2015.

Announcing New CHIE Data Source!

UHIN is pleased to announce that Wasatch Peak Family Practice is contributing laboratory data to the cHIE as of July 12, 2016! Wasatch Peak is a four-physician practice in Layton that offers adult and pediatric primary care, as well as specialty services such as asthma and diabetes care.

Two of their physicians also serve as hospice medical directors, so that they can continue to care for long-time patients who are nearing the end of their lives. Wasatch Peak will enable CHIE users to gain valuable insights into the health of patients of all ages! Their data will display in the Clinical Portal’s Patient Summary view.

Detailed Patient Summaries Now Available in the CHIE

UHIN is pleased to announce that Continuity of Care Documents (CCDs, also called patient summaries) from Mountain West Gastroenterology are now available in the CHIE Clinical Portal! As of June 15, 2016, Mountain West will automatically send CCDs to the CHIE for all of their patients. The data will remain available in the Patient Summary view.

Mountain West Gastroenterology’s CCDs contain a wide variety of in-depth patient information such as allergies, medications and notes. Their CCD information is automatically added to the correct section in the Clinical Portal’s Patient Summary view. For example, family history, patient vital signs, and procedures are now accessible as soon as you pull up a patient. Additional details, such as the text of provider notes, may also be available under the “Documents” tab. To view these details, click on the CCD you’d like to view and choose the HTML option.

The Patient Summary view will also display available information from hospitals participating with Quality Health Network in western Colorado and the Arizona Health-e Connection. CCDs from Intermountain Healthcare, Lone Peak Hospital, St. Mark’s Hospital, and the Veterans’ Affairs Health Care System can be accessed by clicking on the External Documents tab. We are actively testing with more data sources, and expect to include CCDs from a range of providers in the near future.

CCDs provide crucial information about a patient’s health in a clear, easy-to-read format, simplifying transitions and coordination of care. CCD inclusion is one more way we’re making the CHIE more valuable for you.
