Uintah Basin Medical Center Joins the CHIE

UHIN is pleased to welcome Uintah Basin Medical Center to the CHIE! As the CHIE’s newest data source, the 49-bed hospital in Roosevelt, Utah is utilizing CHIE Alerts, to notify providers when their patients have encounters at Uintah Basin’s inpatient facility or emergency department. Additionally, as of August 29, 2016, Uintah Basin contributes general laboratory information to the Clinical Portal, and is actively working with UHIN to send radiology reports and transcription notes in the near future.

As the only hospital within 25 miles of Roosevelt, Uintah Basin plays a crucial role in coordinating care and enabling seamless hospital-to-home transitions for area residents. CHIE Alerts enable providers to follow up with their patients in a timely manner after discharge, reducing the risk of readmissions and complications. UHIN is working with Uintah Basin Medical Center and area clinics to make transitions of care smoother by connecting more clinics to receive CHIE Alerts and locate patient data in the Clinical Portal.

Current users of CHIE Alerts can receive Uintah Basin’s Alerts without any changes to your existing setup. In addition to Uintah Basin, CHIE Alerts are now available from 18 other hospitals throughout Utah, as well as 27 Colorado hospitals and 14 Arizona hospitals through the Patient Centered Data Home initiative, allowing information to follow the patient no matter where they are in the care continuum.

If you would like to receive CHIE Alerts for your patients, please contact us at chie@uhin.org to get started.

Major New Data Sources for the CHIE!

Authorized CHIE users can augment their patient records with data from important new sources! As of February 5, 2016,data is available from Intermountain Healthcare’s 22 hospitals and over 185 clinics in Utah and Idaho, and from the Veterans’ Affairs Health Care System, including emergency services and 10 community clinics in Utah, Idaho and Nevada. Additionally, St. Mark’s Hospital and Lone Peak Hospital are sharing more detailed patient records in the CCD (Continuity of Care Document) format.

It’s simple to locate these partners’ records – just search for your patients as you normally do. Data from Intermountain, the VA and the two HCA hospitals will appear in the patient summary view. Data is automatically placed in the appropriate section for each data type (allergies, lab tests, etc.).

These partners’ information will be available in the patient’s record for 4 hours. However, data for a given patient can be accessed an unlimited number of times by searching for the patient in subsequent queries.

Intermountain, the VA, St. Mark’s and Lone Peak will provide the information available in a standard CCD, including demographic data (name, sex, date of birth, race, ethnicity, and preferred language), problems, medications, allergies, laboratory values/results, vital signs, care plans, immunizations and procedures.

UHIN continually works to make the CHIE a more valuable tool for your office. We are excited about this tremendous progress towards helping every patient get the right care at the right time. This is also an important step in improving care coordination and quality for patients who are medically underserved.

If you have questions about accessing this data, please contact your dedicated UHIN Consultant.
