Now providing valuable information to the clinical Health Information Exchange are: Provo Allergy and Asthma Clinic, and LTPAC providers: Horizon Home Health and Hospice, Dignity Home Health and Hospice, and Mt. Ogden Health and Rehabilitation Center.
