Did you know that you can view medication history in the cHIE for your patients who have had prescriptions paid for by Utah Medicaid? Simply search for a patient, then click on the “Medications” tab in the patient summary view to see a list of medications going back to October 2014. Click the name of a medication to see the fill date, dosage and format (solution, etc.).
Support for medication reconciliation is one more way that the cHIE supports patient safety and care coordination.
