New SNFs contributing valuable data to the clinical Health Information Exchange are: Harrison Pointe Healthcare and Rehabilitation, Holladay Healthcare Center, and St. Joseph Villa. Joining these organizations are home health and hospice entities Community Nursing Services, and OneCare Home Health and Hospice.
